Affidavit of Heirship Family Lines - Find Your Place
When completing an Affidavit of Heirship, use this form to figure out your line of heirship to best determine who is responsible for completing the Affidavit of Heirship, and who must complete a Relinquishment of Rights form.

Relinquishment of Rights
This form is to be used when relinquishing (giving up) rights to a grave site. This form is generally used in conjunction with an Affidavit of Heirship when one family member is giving all rights to another family member.
Interment Authorization for Relative (Aunt, Uncle, Cousin, Grandparent)
This form is to be used to authorize a relative (Aunt, Uncle, Cousin, Grandparent) to utilize a grave site originally owned by an individual's parent.
Sibling Interment Authorization
This form is for a sibling of a deceased to authorize interment to one of their siblings in a lot originally owned by their mutual parents.
